Community Survey

Every 3 years, CAPSAW undertakes a community needs assessment process.  The purpose of this assessment is to create a profile of our community's needs, resources, service gaps and aide in program evaluation.    

CAPSAW's process will include the use of existing data already compiled by local, state and federal stakeholders.  This information will be analyzed along with data generated through a combination of  surveys, community conversations and key informant interviews conducted by CAPSAW and through community partners.. 

The resulting Community Needs Assessment Report will be used to help evaluate our progress towards the goals outlined in our Strategic Plan.  Your voice is an important part of this process.  Please complete the 2017 survey by following this link:
